Position Details

• Location Gastonia NC 28054 on site
• Coverage Area Gaston County Lincoln County Cleveland County and zip code 28214
• Pay Rate $48.69 per hour
• Schedule Full Time Monday through Friday 8:00 AM to 5:00 PM
• No weekends and no on call requirements
• Weekday holidays may be required

What You Will Do

• Assess plan implement and evaluate individualized nursing care plans
• Provide case management focused on health maintenance and disease prevention
• Coordinate care with physicians providers and interdisciplinary teams
• Manage medication processes including ordering monitoring documentation and proper disposal
• Maintain accurate and timely nursing documentation across required reporting periods
• Communicate changes in patient condition to physicians and care teams
• Transcribe physician orders and medically related reports as needed
• Coordinate care during hospitalizations and discharges
• Supervise Licensed Practical or Vocational Nurses as required
• Train and onboard new employees and provide ongoing health and safety education
• Ensure infection control standards and proper maintenance of equipment and supplies
• Schedule and follow up on patient health related appointments

What We Are Looking For

• Active Registered Nurse license
• Eligible for or willing to work under a Compact RN license
• Commitment to required attendance during the first 90 days
• Ability to work independently in a field based onsite environment
• Strong documentation communication and care coordination skills
• Home health experience preferred
• Experience with Homecare Homebase wound care wound vac IV therapy and port access preferred
