Home Health Registered Nurse (RN)
Location: Grand Rapids, MI (Field-Based; territory within ~60 miles)
Schedule: Monday–Friday (daytime visits; flexible based on patient needs). On-call may be required.
Pay: $57.23–$62.00/hr
Summary
Hiring a Home Health Registered Nurse (RN) to provide skilled in-home nursing care in the Grand Rapids, MI area. This Home Health RN role includes comprehensive assessments, Plan of Care (POC) management, and accurate OASIS documentation using Homecare Homebase (HCHB). If you have strong clinical skills (wound care, IV care) and enjoy independent field work, this Home Health Registered Nurse role offers excellent pay and consistent daytime scheduling.
Responsibilities
- Perform skilled nursing visits: assessments, reassessments, and care coordination in patient homes
- Develop and evaluate Plans of Care (POC) and document changes clearly
- Complete accurate OASIS documentation and charting in HCHB (Homecare Homebase)
- Provide wound care, IV care/venipuncture, catheter/drain care, and medication education
- Educate patients and caregivers on disease management, home safety, and self-care
- Coordinate with physicians and interdisciplinary teams; ensure continuity of care
- Ensure compliance with infection control (OSHA), HIPAA, and payer documentation requirements
- Travel across assigned territory (approximately a 60-mile radius) and participate in on-call coverage as assigned
Qualifications
- Active, unrestricted Registered Nurse (RN) license (Michigan)
- Current CPR/BLS certification
- 2+ years home health experience preferred
- Strong HCHB experience required and strong OASIS experience required
- Valid driver’s license, reliable transportation, and ability to travel daily
- Strong assessment skills, documentation accuracy, and time management
- CHPN certification preferred (not required)
