LMSW Compensation & Highlights

• Hourly Rate: $46.00-$56.00/hour, paid biweekly
• This position is a W2 contract assignment with potential for extension and/or conversion to direct based on attendance, performance, and business needs.
• Target start: Likely 03/31/2025
• Coverage Area: Brooklyn Park, Minnesota
• Expected Weekly Productivity/Units: 20 units per week on average

  • Oasis Recert: 1.75 units
  • Eval: 1.5 units
  • Regular visit: 1.25 units

LMSW Responsibilities

• The Medical Social Worker participates in the interdisciplinary care provided to home health patients. Functions to evaluate and develop a plan of care personalized to fit the patient's emotional and social needs. Provides direction and supervision of the Social Worker Assistant as required and when involved in the patient's plan of care. Works within company specific policy and procedures, applicable healthcare standards, governmental laws and regulations.
• Evaluates patients' social and emotional well-being related to their illness, care needs, and treatment response.
• Assesses home environment, financial status, and community resource availability.
• Connects patients with community resources to support recovery.
• Counsels patients and families on coping with illness-related social and emotional challenges.
• Aids healthcare team in understanding social/emotional factors affecting patient health.
• Contributes to care planning and re-evaluation with physicians.
• Monitors and reports patient condition changes to Clinical Manager and physician.
• Engages in interdisciplinary care coordination and supports care team needs.
• Supervises, instructs, and evaluates Social Work Assistants (BSW).
• Completes required documentation, including case notes and care coordination records.

LMSW Requirements

• Homecare Homebase: 1 years
• Home Health Care: 1 years
• OASIS (Outcome and Assessment Information Set): 1 years
• Current Licensure/certification in Minnesota - Required
• Current Driver's License; must have reliable transportation and provide proof of insurance - Required
• Master's or doctoral degree from a school of social work accredited by the Council on Social Work Education - Required
