Hospice RN Case Manager Compensation

• The pay for this position is $56.14 per hour
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Hospice RN Case Manager Highlights

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• The required availability for this position is Monday – Friday, 8:00 AM – 5:00 PM with an on-call rotation (evenings, overnight, 24-hour shifts, or entire weekends as needed)
• Typically 5 visits per day

Hospice RN Case Manager Responsibilities

• Perform comprehensive assessments of high-risk members
• Collaborate with primary care providers to implement individualized, comprehensive care plans with targeted interventions
• Continuously reassess cost-efficient and appropriate levels of care
• Arrange non-medical support services such as housing and transportation to ensure treatment plan compliance
• Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals
• Engage specialty resources as needed to achieve optimal patient outcomes
• Maintain detailed records of clinical, functional, and fiscal outcomes during case management
• Identify opportunities for health promotion and illness prevention
• Prevent adverse patient occurrences when possible and intervene quickly if necessary
• Act as a patient advocate, ensuring privacy and confidentiality

Hospice RN Case Manager Requirements

• Current unrestricted Registered Nurse (RN) license – Required
• Associate’s or Bachelor’s Degree in Nursing or related field – Required
• Case Management certification – Preferred
• HCHB (Home Care Home Base) experience – Highly Preferred
• If no HCHB experience, candidates must be tech-savvy
• High School Diploma or GED
• Attendance is mandatory for the first 90 days
