Compensation

• The pay for this position is $63 - $67 per hour

• Benefits are available to full-time employees after 90 days of employment

• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Highlights

• This position is a contract assignment with potential to hire on permanently based on attendance, performance, and business needs

• Schedule: Monday - Friday, 8:00 AM - 5:00 PM (Overtime & weekends as needed based on patient availability)

• Department: Homecare Hospice & Palliative Care

Responsibilities

• Perform patient assessments to determine hospice eligibility and care needs.

• Develop and implement individualized care plans for patients and families.

• Provide education and support for patients and caregivers.

• Manage and prioritize patient care needs while ensuring proper case management.

• Ensure compliance with hospice regulations and insurance reimbursement guidelines.

• Attend interdisciplinary team (IDT) meetings and collaborate with healthcare teams.

• Maintain accurate and timely documentation in EMR systems.

• Supervise home health aides (HHA) to ensure quality patient care.

• Support agency growth through mentorship and training of new employees.

Qualifications

• Required:

  • 2-3 years of experience as an RN in a home hospice setting.

  • Active New Jersey RN License.

  • BLS Certification.

  • Experience using an EMR system.

  • Strong critical thinking and communication skills.

  • Valid New Jersey driver’s license.
