Hybrid Behavioral Case Manager
Looking for a Social Work Case Manager -No License Required, Hybrid Remote and in Field
Hours: Monday – Friday, 8am-5pm, no weekends, no holidays
Pay: $34.13
Travel in County Required:
• Travel time is paid at the hourly wage
• Mileage is reimbursed at the IRS rate
• Must live in MACON COUNTY or immediate surrounding area
Hybrid Behavioral Case Manager Job Duties:
• Coordinates case management activities for Medicaid Long Term Care/Comprehensive Program enrollees.
• Conducts comprehensive evaluation of Members using care management tools and information/data review
• Coordinates and implements assigned care plan activities and monitors care plan progress
• Conducts multidisciplinary review to achieve optimal outcomes
Hybrid Behavioral Case Manager Requirements:
• Case Management experience required
• Bachelor’s Degree Required (Social Work or related field)
• Willing and able to travel using your own vehicle 50-75%
of their time to meet with members face to face in Macon County and surrounding areas (Must have reliable
transportation and a drivers license)
Hybrid Behavioral Case Manager Preferred Qualifications:
• Long Term Care experience preferred
• Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
• Effective communication skills, both verbal and written
