Hybrid Behavioral Health Care Manager (LSW , LPC, LMTH, LMHC, LCSW)


Target Start Date: ASAP (March 2 preferred)
Work Schedule: Monday–Friday, 8:00 AM – 5:00 PM EST
Pay/Bill Rate: $43.00-44.00

Location: Remote – Ohio (must reside in assigned regions)

Regional Assignments

  • Northeast Ohio: Medina, Cuyahoga, Lake, Lorain, Geauga
  • Northwest Ohio: Lucas, Fulton, Wood, Ottawa
  • West Central Ohio: Montgomery, Greene, Clark

 

Position Overview

The Care Manager develops, assesses, and facilitates complex care management activities for members with primarily physical health needs. This role focuses on delivering high-quality, cost-effective healthcare outcomes through personalized care plans, education, and coordination of services.

Care Managers work collaboratively in a supportive team environment to ensure members receive the care and services needed to remain as independent as possible.

 

Typical Day & Responsibilities

  • Log in by 8:00 AM to review emails, tasks, and voicemails
  • Prioritize and respond to urgent member needs
  • Conduct 5–7 member visits per week (in-field approximately 50–60% of the time)
  • Complete documentation from home when not in the field
  • Maintain documentation completion within 24 hours of each visit
  • Participate in scheduled meetings and trainings
  • Coordinate care plans, provide member education, and support care transitions

 

Training Schedule (Mandatory)

  • Duration: 4 weeks
  • Schedule: Monday–Friday, 8:00 AM – 5:00 PM EST
  • Format: Virtual classroom and 1:1 training
  • Requirements:
    • Cameras ON at all times
    • No time off permitted during training

 

Performance Expectations

  • Complete 5–7 member visits weekly
  • Meet required turnaround times for documentation and processes
  • Complete all assigned trainings on time
  • Demonstrate strong time management, independence, and accountability

Required Qualifications

 

Education & Licensure (Required)

  • Master’s degree in a Behavioral Health–related field
  • Active Ohio licensure: LSW, LCSW, LMSW, LMFT, LMHC, or LPC

Experience

  • 2–4 years of related experience in:
    • Case management or home health
    • Discharge planning
    • Behavioral health

 

Required Skills (Top 3)

  1. Computer literacy (Microsoft Excel, Word, Teams, Outlook, OneNote, OneDrive, PowerPoint)
  2. Critical thinking and clinical judgment
  3. Ability to work independently
