It would be working remote 25-50% of the time and in the field doing visits to members homes (or Long Term Care facilities) 50-75% of the time. Must live and be wiling to travel with in one of the areas listed:

southwest region - Van Buren, Kalamazoo, Calhoun, Berren, Cass, St. Joseph, and Branch Counties.

Hybrid Nurse Case Manager JOB SUMMARY:

· Assess the need for case management intervention by obtaining and objectively evaluating data, including clinical information, medical history, level and location of care, functional status, psychosocial and environmental factors

· Documents assessment, plan of care and timetable for implementing interventions

· Facilitates the implementation of the care plan by optimizing available benefit, comparing the benefits and cost of care.

· Plan alternatives based on recipient needs and investigate opportunities to use available community resources and educates the recipient, family or significant other, physician and providers relating to the availability of community resources and services

· Work closely with the PCP, specialists and other providers to support the treatment plan and achievement of goals developed for and by the recipient or caregiver;

· Measure effectiveness, necessity and efficiency of the care plan and make revisions as needed

· Collaborates with enrolled providers to assure recipients are referred to the appropriate providers to deliver care and services to recipients in a cost effective manner

Hybrid Nurse Case Manager HIGHLIGHTS:

· Mon-Fri, 8a-4:30p (no nights or weekends) Travel time included in 8-4:30 hours.

· Benefits are available to full-time employees after 90 days of employment and include health, PTO, 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

· The compensation for this position is $44.14

Hybrid Nurse Case Manager Requirements:

· Possess an active MI Registered Nurse license

· 3 years of Clinical Experience (Medical surgical, Behavioral Health, Etc.)

· Case Management experience preferred

· Home Health/Home Care experience a plus

· Computer savvy
