Hybrid RN Case Manager Compensation

• The pay for this position is $33.52 per hour

Hybrid RN Case Manager Highlights

• Full time: 40+ hours per week
• Shift/Hours: Monday-Friday 8am-5pm

Hybrid RN Case Manager Responsibilities

• Coordinates and implements assigned care plan activities and monitors care plan progress.
• Evaluation of Members: -Through the use of care management tools and information/data review, conducts comprehensive evaluation of referred member’s needs/eligibility and recommends an approach to case resolution and/or meeting needs by evaluating member’s benefit plan and available internal and external programs/services.
• Utilizes critical thinking and judgement to collaborate and inform the case management process, in order to facilitate appropriate healthcare outcomes for members by providing care coordination. Support and education for members through the use of care management tools and resources.
• Identifies high risk factors and service needs that may impact member outcomes and care planning components with appropriate referral to clinical case management or crisis intervention as appropriate.

Hybrid RN Case Manager Requirements

• Bachelor or Associate of science in nursing required
• Active state VA RN license required
• Case management and discharge planning experience preferred
• At least 1 year of experience as a registered nurse
