LPN [Licensed Practical Nurse] Continuing Care Compensation

• The pay for this position is $38.07-$43.51 per hour
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

LPN [Licensed Practical Nurse] Continuing Care Highlights

• 2nd Shift: 2:30p-11:00p
• 8-Hour Shift
• Rotating weekends
• This position is a W2 assignment with potential to hire on permanently based upon attendance, performance, and business needs.
• The required availability for this position is full time.

LPN [Licensed Practical Nurse] Continuing Care Responsibilities

• Under the direction of the RN / Mid-level Provider / Physician, contributes to the assessment planning implementation and evaluation of patient care.
• Provides direct patient care under the direction of the RN/Mid-level Provider/Physician consistent with policies and procedures and unit based routines
• Documents care provided via computer/written patient record in a manner consistent with guidelines.
• Cleans patient's environment/work area/equipment and communicates any equipment malfunctions to appropriate department/unit personnel.
• Advocates for privacy confidentiality and security of patient staff and organization data.

LPN [Licensed Practical Nurse] Continuing Care Requirements

• Candidates must be willing to float between all continuing care spaces.
• Fingerprints and AHA
• Required Associate's Degree Graduate of an approved nursing program
• Must have at least 1 year of LPN experience.
• Attendance is mandatory for the first 90 days

LPN [Licensed Practical Nurse] Continuing Care Preferred Qualifications

• High School Diploma or GED
• Healthcare Experience Preferred
• Neuro Residential 2 Yrs. Preferred
• LIC - Driver's License
• CRT
• BLS
