HOME HEALTH LPN | DETAILS AND COMPENSATION:
- Location: Carmel IN 46032 – 100% On-site, home health settings
- Payrate: $37.30/hr + mileage
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
HOME HEALTH LPN | SUMMARY AND HIGHLIGHTS:
The Home Health LPN will provide high-quality home health care services to clients in the Greater Indianapolis area. This role focuses on patient-centered care, strong documentation, and collaboration with interdisciplinary care teams to support patient independence and dignity.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
HOME HEALTH LPN | RESPONSIBILITIES:
- Provide nursing care in accordance with each client’s individualized plan of care
- Monitor vital signs and report abnormal findings promptly
- Administer medications, feedings, oxygen, and perform ostomy care as ordered
- Complete routine care established by PT, OT, and Speech Therapy providers
- Maintain strict infection control standards and report potential communicable diseases
- Accurately document all nursing care, patient progress, and outcomes
- Order and maintain adequate supplies, medications, and nutritional support
- Ensure patient needs are met per physician instructions
- Monitor and respond to patient incidents; complete incident reports accurately and timely
- Inspect medical equipment and report faulty items for repair
HOME HEALTH LPN | REQUIREMENTS:
- Education: High School Diploma or GED (verifiable)
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active Indiana LPN license or compact license
- Home health experience required
- Strong patient relations and communication skills
- Proficiency with electronic documentation systems
- Ability to use Microsoft Teams and Outlook
- Reliable transportation (mileage reimbursed)
