LTC Closed Door Pharmacist – Bethlehem, PA

Pharmacist Compensation

• Pay rate: $60.00 per hour
• Benefits available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with one year of service on our eligibility dates

Pharmacist Highlights

• Contract assignment with the potential for permanent hire based on performance, attendance, and business needs
• Shift: 7:00 PM – 3:30 AM (Must be able to work weekends)

Pharmacist Responsibilities

• Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration
• Make recommendations for therapy changes to physicians and facility staff as needed
• Maintain patient database including medical history, clinical and laboratory data, and current medications
• Ensure appropriate laboratory tests are ordered, evaluated, and discussed with the healthcare team for high-risk conditions
• Collaborate with the healthcare team to provide clinical and technical support
• Document communication with physicians, nurses, healthcare team members, and patients/residents in medical records
• Implement policies and procedures for the safe procurement, storage, distribution, and disposal of medications
• Obtain and maintain all required documentation per federal and state regulations
• Report medication-related errors to the Pharmacy Manager
• Assist facility staff with safe and effective medication management procedures
• Perform other related tasks as assigned

Pharmacist Requirements

• Bachelor’s Degree in Pharmacy; PharmD preferred
• One year of experience in long-term care
• Active Pennsylvania Pharmacy License (required)
• Strong attention to detail, goal-oriented, and a team player
• Excellent oral communication skills
• Attendance is mandatory for the first 90 days
