Job Description
A-Line Staffing is now hiring RN Case Manager in New Castle County, Delaware. The RN Field Care Manager would be working for a Major Fortune 500 Company and has career growth potential.
RN Case Manager Highlights:
- Schedule: Monday – Friday, 8:00 AM – 5:00 PM EST No overtime expectations
- Pay Range: $36-40 Hourly, plus mileage reimbursement
RN Case Manager Responsibilities:
- Complete LTSS Level of Care (LOC) redeterminations for members.
- Conduct member home visits and care assessments.
- Support LTSS case managers with case management activities.
- Manage transition of care activities.
- Evaluate older adults and disabled individuals to determine level of functioning.
- Perform medication reconciliation as part of member assessments.
- Maintain compliance with LTSS program requirements and redetermination timelines.
- Assist members with immediate care and service needs.
- Document assessments, care plans, and case management activities accurately in system platforms.
RN Case Manager Qualifications:
- Associate's or Bachelor's Degree required.
- 2–4 years of related healthcare, case management, or LTSS experience required.
- Active Delaware RN License or Compact RN License required.
- Valid Driver’s License required.
- Must reside within Delaware.
- Home workspace required.
- Level of Care Redeterminations.
- Transition of Care.
- Case Management.
- Evaluating older adults and disabled individuals for functional status.
- Medication reconciliation.
- Strong computer literacy skills.
Preferred Qualifications:
- Previous experience as a Service Care Manager.
- LTSS program experience.
