Job description:

We are looking for compassionate and dedicated Licensed Practical Nurses (LPN) to join healthcare teams in Philadelphia, PA 19136 at a correctional facility. These positions involve working with a diverse patient population in a correctional healthcare setting on 3rd shift (11pm - 7:30am), with both full-time and part-time options available. You will play a crucial role in providing direct nursing care, patient education, and collaborating with medical teams to deliver high-quality healthcare.

LPN Schedule & Location:

• Full-Time: 11pm - 7:30am, guaranteed 40 hours per week, every other weekend.
• Part-Time: 11pm - 7:30am, guaranteed 16 hours per week, every other weekend.
• Both positions are located in Philadelphia, PA 19136 at a correctional facility.

LPN Salary/Wage:
$35.00 - $40.00/hr, depending on experience.

LPN Duties:

• Contribute to the assessment of patients’ health status.
• Participate in developing and modifying patient care plans.
• Provide safe and effective nursing care within the scope of practice.
• Perform delegated tasks consistent with prior education and training.
• Educate patients based on established care plans.
• Monitor patient care processes and ensure follow-up, including telemedicine appointments.
• Work collaboratively with healthcare teams, including medical directors, nursing staff, and other leaders.
• Participate in quality improvement initiatives and patient care committee meetings.
• Ensure the coordination of patient care across both onsite and offsite settings.

LPN Required Qualifications:

• Graduate from an accredited Licensed Practical Nursing (LPN) program.
• At least 1 year of LPN experience.
• Currently licensed as a Practical Nurse in Pennsylvania.
• Active CPR certification.
• Knowledge of state laws and regulations governing nursing practice.
• Background check, security clearance, and drug screening required.

LPN Physical Requirements:

• Ability to perform routine nursing duties, including lifting, bending, and standing for extended periods.
• Capacity to assist with patient mobility and other physical aspects of patient care.

LPN Education:
Graduate from an accredited Licensed Practical Nursing (LPN) program.
