Compensation
- The pay for this position is $40.00 per hour
- Benefits are available to full-time employees after 90 days of employment
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Highlights
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- The required availability for this position is standard business hours, Monday–Friday (40+ hours/week)
Responsibilities
- Coordinate and process Long-Term Care admissions, transitions, and authorizations
- Manage and track bed holds, leave of absence requests, and service authorizations
- Process supportive services including DME, transportation, and therapies
- Issue member and provider notifications via phone, fax, mail, and electronic systems
- Collaborate with hospitals, SNFs, providers, vendors, and internal clinical teams
- Ensure compliance with Title 22 LTC guidelines and regulatory requirements
- Run utilization reports and monitor authorization trends and “hold” status
- Support eligibility verification, claims issues, appeals, and grievances
- Communicate benefit and authorization information to providers and members
- Maintain accurate documentation across multiple systems and platforms
Requirements
- Bachelor’s degree in healthcare OR
- Associate degree + 1 year healthcare experience OR
- High School diploma + equivalent healthcare experience
- 1+ year experience in managed care or healthcare setting preferred
- Experience with Medicare and Medi-Cal populations
- Knowledge of DMHC regulations and DHCS requirements
- Experience in utilization review, SNF, discharge planning, or home health
- Strong computer skills including Microsoft Office and Windows-based systems
- Excellent verbal and written communication skills
- Valid driver’s license, clean driving record, and auto insurance required
- High School Diploma or GED
- Attendance is mandatory for the first 90 days
Preferred Qualifications
- Experience in Skilled Nursing Facility (SNF) or long-term care settings
- Utilization management or care coordination experience
- Knowledge of community-based services and behavioral health coordination
- Experience working with health plan authorization workflows
