Long Term Care Coordinator
Location: Remote (California residents only)
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Pay: $40.00/hr
Summary
California Residents only. Seeking a Long Term Care Coordinator to support long-term care authorization processing and documentation tracking for admissions and transitions. This role focuses on accurate, high-volume coordination with SNFs, hospitals, delegated partners, vendors, and internal clinical teams, ensuring authorizations and supporting documentation are complete, timely, and compliant. The ideal Long Term Care Coordinator has strong knowledge of Medi-Cal/Medicare, understands Title 22 processes, and is comfortable communicating benefits/eligibility and resolving issues tied to authorizations, claims, appeals, and grievances.
Responsibilities
- California Residents only
- Process and track LTC authorizations, admissions, transitions, bed holds, and LOA requests
- Coordinate supportive services (DME, transportation, therapies) and issue required notifications
- Communicate with SNFs/hospitals/providers and internal teams to resolve documentation and authorization issues
- Run utilization reports and monitor “hold” status authorizations and trends
- Support eligibility/benefit verification and respond to provider/member/staff inquiries
- Perform accurate documentation and data entry across multiple systems
Qualifications
- California Residents only
- HS diploma/GED plus healthcare experience, or AA/AS/college, or Bachelor’s in healthcare-related field
- 1+ year managed care/healthcare experience preferred
- Experience with Medi-Cal/Medicare, DMHC, DHCS, and LTC/SNF workflows strongly preferred
- Strong computer skills (Windows + Microsoft Office), typing/data entry accuracy, and professional communication
- Valid driver’s license + auto insurance required
