Job Description:
We are seeking a dedicated and experienced Long Term Care Nurse Specialist for a field-based position in New York, New York. This role is primarily focused on conducting member assessments in the home setting. The nurse will be responsible for completing 2-5 assessments per day, depending on the type and complexity of each visit. Critical thinking skills will be essential to assess care needs, identify any issues, and document findings accurately. The nurse will review electronic medical records (EMR) and previous assessments before each visit, ensuring a comprehensive approach to care. This is a flexible position requiring travel, adaptability to varying weather conditions, and the ability to manage a dynamic schedule.
Daily Activities Include:
- Reviewing EMR and previous assessments in preparation for each visit
- Conducting member assessments in their homes
- Using critical thinking to evaluate care needs and document findings
- Completing and uploading assessment documentation using provided templates
- Adapting to varying weather conditions and locations, ensuring flexibility in managing each day's schedule
- Completing 2-5 assessments per day based on the type and complexity of the visits
Long Term Care Nurse Specialist Schedule & Location:
- Shift: 8:30 AM – 5:00 PM (7.5 hours daily)
- Location: Field-based in Brooklyn, NY, Bronx, NY, or Manhattan, NY
- Overtime: Overtime may be required based on workload
Long Term Care Nurse Specialist Salary/Wage:
- $55 - $65 per hour, based on experience
Long Term Care Nurse Specialist Duties:
- Conduct home assessments to evaluate the care needs of members
- Use critical thinking to address and solve challenges identified during assessments
- Review EMR and previous assessments to prepare for visits
- Complete and document assessments using provided templates and tools
- Travel to members’ homes, adapting to different locations and conditions
- Complete 2-5 assessments per day depending on the type and complexity of the visit
Long Term Care Nurse Specialist Required Qualifications:
- RN license in New York State
- NY State Driver’s License
- UAS certification
- Experience in MLTC, MAP, or Maximus programs
Long Term Care Nurse Specialist Physical Requirements:
- Ability to travel between various locations and handle changing weather conditions
- Must be adaptable to various home environments and patient needs
Long Term Care Nurse Specialist Education:
- Required: Associate's Degree (AS)
- Preferred: Bachelor of Science in Nursing (BSN)
