Long Term Care Pharmacist
Job Description
Pay: $64.27 per hour
Location: Fridley MN 55421
Job Type: 3-month contract to hire | Onsite
Shifts:
- Monday – Friday: 9:30 AM – 6:00 PM / 10:00 AM – 6:30 PM / 11:00 AM – 8:30 PM
- Saturday – Sunday: 10:00 AM – 6:30 PM
- Overtime not required but may be offered occasionally
Overview:
The Pharmacist is responsible for safely dispensing prescription medications, monitoring patient medication compliance, maintaining accurate records, providing patient consultations, and supervising pharmacy staff. This role ensures proper management of patient records, adherence to legal requirements, and delivery of high-quality patient care. IV experience is a plus but not required.
Duties & Responsibilities:
- Accept and dispense prescription medications according to state and federal regulations
- Monitor patient medication compliance and detect over-usage patterns
- Maintain accurate, confidential patient records and pharmacy files
- Advise patients on medications, including dosage, side effects, drug interactions, and proper use
- Provide consultations on prescription and non-prescription medications
- Educate patients on medical device usage, such as blood pressure monitors and glucose meters
- Compound medications for external or internal use, including sterile solutions following proper procedures
- Supervise pharmacy staff including pharmacy technicians, clerks, and interns
- Maintain appropriate inventory of pharmaceutical and medical supplies
- Identify drug-related problems and communicate effectively with physicians and healthcare teams
- Collaborate with physicians, nurses, other pharmacists, and interns to ensure comprehensive patient care
Qualifications:
- Doctor of Pharmacy (PharmD) degree required
- Active pharmacy license issued by the state pharmacy board
- IV experience is a plus but not required
- Strong knowledge of medication safety, regulations, and patient counseling
- Excellent organizational, communication, and leadership skills
- Ability to manage multiple priorities in a fast-paced environment
Physical & Work Environment:
- Ability to perform standard pharmacy duties including standing for long periods, lifting up to 30 pounds, and performing repetitive tasks
- Work in a fast-paced, patient-centered environment
