MTM Clinical Pharmacist Job Description:
This is a clinical (non-retail) pharmacy role focused on Medication Therapy Management (MTM) and direct patient interaction. The MTM Clinical Pharmacist will provide structured counseling on new drug therapies in accordance with clinical management models and perform detailed pharmacotherapy reviews, including medication utilization, side effect profiling, drug interaction analysis, and adherence monitoring. The pharmacist will play a vital role in optimizing patient treatment plans in collaboration with interdisciplinary teams, while ensuring full compliance with clinical and regulatory standards.


MTM Clinical Pharmacist Schedule & Location:
• Monday – Friday, 10:00 AM – 7:00 PM CST
• Onsite – Must live within one hour of Fort Worth, TX 76177


MTM Clinical Pharmacist Salary/Wage:
• $60 per hour


MTM Clinical Pharmacist Duties:
• Conduct comprehensive clinical pharmacotherapy reviews for each patient
• Screen for and manage drug interactions, side effects, and therapy compliance
• Counsel patients on new drug therapies using clinical management models
• Document clinical findings and patient education in the medical record
• Coordinate care with physicians and interdisciplinary care teams
• Serve as liaison between patients, caregivers, nurses, and prescribers
• Provide subcutaneous medication training and support for patients/caregivers
• Process prescriptions and accurately assign therapy outcomes
• Perform intake for new referrals and document accordingly
• Adhere to REMS training protocols and internal SOPs
• Implement clinical management models for complex disease states
• Dispense medications per federal/state law and board of pharmacy regulations
• Supervise pharmacy technicians and aseptic procedures
• Ensure accurate adjudication of pharmacy claims
• Monitor hazardous waste protocols within pharmacy operations
• Participate in adverse event reporting and internal QA tracking
• Promote constant learning and attend pharmacy-related conferences
• Support the professional development of pharmacy staff


MTM Clinical Pharmacist Required Qualifications:
• Active and unrestricted pharmacist license in TX or NC
Minimum 3 years of clinical counseling experience (not retail)
• Proven experience conducting comprehensive pharmacotherapy reviews
• 3–5 years of post-graduate clinical pharmacy experience
• Ability to handle high-volume patient counseling via phone (8 hrs/day)
• Excellent communication, documentation, and multi-tasking skills
• Willing to obtain multi-state licensure as needed
• Strong computer and pharmacy system knowledge
• Familiar with sterile compounding and quality assurance practices


MTM Clinical Pharmacist Physical Requirements:
• Must be able to sit and talk on the phone for extended periods (up to 8 hours/day)


MTM Clinical Pharmacist Education:
• Graduate of an accredited school of pharmacy
• BCOP certification or willing to obtain when eligible
• Meets or exceeds current CE requirements per Board of Pharmacy regulations
