This position operates on a Monday to Friday schedule with 8-hour shifts between 7am - 5:30pm Mountain Time.

Compensation: $50 - $52 per hour, based on licensure, certification, and experience.
Description:
As a Case Manager with the Department of Veterans Affairs, your efforts will directly contribute to improving the lives of veterans by coordinating comprehensive care and providing crucial education about health conditions, diseases, and illnesses. Your work will empower these individuals and their families, ensuring they are informed, supported, and cared for with compassion.
Key Responsibilities:

Coordinate and Provide Patient Care: You’ll be the go-to resource for veterans, working alongside the VA’s Referral Coordination Team to provide seamless, patient-centered care.

Educate Veterans & the Public: You will help veterans and their families understand their health conditions, providing the education they need to take control of their care.

Administrative Support: Perform vital administrative duties within the VA’s Office of Community Care and Referral Coordination Team, ensuring processes run smoothly and efficiently.
Minimum Qualifications:

Two years of direct patient care experience within the last three years.

Associate’s or Bachelor’s degree in Nursing from an accredited institution.

Must possess and maintain an active, full, and unrestricted Registered Nurse license to practice in the United States or the District of Columbia.

Active BLS (AHA) certification.

Proficiency in English, including reading, writing, speaking, and understanding.

Basic computer skills, including typing.
