Nurse Case Manager II Compensation

• The pay for this position is $40.70 per hour.
• Benefits are available to full-time employees after 90 days of employment.
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates.

Nurse Case Manager II Highlights

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs.
• The required availability for this position is Monday through Friday, 8:00 AM to 5:00 PM EST.
• This is a field-based, patient-facing role with travel required throughout Fulton, Wood, Ottawa, and Lucas Counties
• Fingerprinting is required.

Nurse Case Manager II Responsibilities

• Conduct comprehensive assessments of referred members’ needs and eligibility using clinical tools and data.
• Evaluate member benefit plans and available internal/external services to determine case resolution approach.
• Apply criteria, guidelines, and case management plans to ensure appropriate administration of benefit.
• Utilize case management and quality management processes in compliance with regulatory and accreditation standard.
• Promote optimal, cost-effective outcomes through planning, facilitation, and advocacy.

Nurse Case Manager II Requirements

• RN with current unrestricted state licensure
• Minimum 3 years of clinical practice experience (hospital, home health, or ambulatory care
• Experience in healthcare and/or managed care industry
• Proficiency in computer skills including navigating multiple systems and keyboarding.
• Effective verbal and written communication skill
• Ability to multitask, prioritize, and adapt to a fast-paced environment.
• High School Diploma or GE
• Attendance is mandatory for the first 90 days.

Nurse Case Manager II Preferred Qualifications

• Case Management Certification (CCM)
• Case management experience
• Familiarity with regulatory standards and productivity expectations in office and field environments
