Pay Rate: $74.87/hour
Schedule: Varies but most locations are open during these time frames, must be available for 32-40 hours per week
- Monday–Friday: 8:30 AM – 7:30 PM
- Saturday: 9:00 AM – 5:30 PM
- Sunday: 9:00 AM – 4:30 PM
- Every other weekend required
- Possible holiday rotations
- Work Setting: In-person, patient-facing (onsite as needed)
Position Overview
We are seeking an experienced Family Nurse Practitioner to provide high-quality, evidence-based primary and family care in a fast-paced outpatient clinical setting. This role involves working collaboratively with a dedicated clinical care team to manage a panel of patients and drive positive health outcomes through assessment, diagnosis, treatment, education, and care planning.
Key Responsibilities
- Provide primary and family care including accurate assessment, diagnosis, treatment, and management of acute and chronic health conditions
- Develop and implement individualized care plans, including disposition planning and follow-up
- Perform routine physical exams and diagnostic testing
- Educate patients on healthy lifestyle choices, preventive care, and chronic disease management
- Encourage patient engagement through health screenings, wellness services, and behavioral health support
- Collaborate closely with physicians, nurses, and other healthcare professionals to ensure comprehensive care
- Handle administrative clinic duties including opening/closing procedures, inventory management, patient follow-up, insurance verification, and payment collection
- Maintain a clean, safe, and organized clinical environment
- Utilize electronic medical records and technology efficiently to document patient care
Required Qualifications
- Master’s Degree from an accredited Family Nurse Practitioner program
- Current National Board Certification
- Active, unrestricted Nurse Practitioner license in the state of employment
- Current BLS certification (must be active at time of offer acceptance)
- Verifiable High School Diploma or GED
- Nurse Practitioner Candidate Questionnaire must be included on the resume
Preferred Qualifications
- Minimum of 1 year of recent experience as a Nurse Practitioner in primary care or family medicine
- Strong understanding of quality measures and chronic disease management
- Ability to practice autonomously with minimal supervision
- Excellent verbal, written, and electronic communication skills
- Strong organizational skills with the ability to multitask and prioritize effectively
- Self-motivated, adaptable, and patient-focused
- Proficiency with healthcare technology and information management systems
- Willingness to obtain multi-state licensure
- Ability to collaborate frequently with a remote or hybrid clinical care team
