Job Overview
We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The ideal candidate will provide high-quality outpatient care to patients across all age groups, focusing on preventive health, chronic disease management, and overall wellness. This role requires a strong commitment to patient care and the ability to work collaboratively within a multidisciplinary team. Experience in memory care, sterile processing, and familiarity with Cerner electronic health records is highly desirable.

Responsibilities

  • Conduct comprehensive assessments of patients to develop individualized care plans.
  • Provide primary care services including diagnosis, treatment, and management of acute and chronic illnesses.
  • Perform routine physical examinations and preventive health screenings.
  • Administer vaccinations and other preventive measures in accordance with established standards.
  • Collaborate with physicians and other healthcare professionals to ensure coordinated patient care.
  • Educate patients and their families about health maintenance and disease prevention strategies.
  • Maintain accurate and up-to-date patient records using Cerner or similar electronic health record systems.
  • Participate in quality improvement initiatives aimed at enhancing patient outcomes.
  • Stay current with best practices in outpatient care, critical care, and memory care.

Qualifications

  • Master’s degree in Nursing from an accredited program.
  • Current certification as a Family Nurse Practitioner (FNP) by a recognized certifying body.
  • Valid state nursing license and certification in Basic Life Support (BLS).
  • Experience in outpatient settings preferred; familiarity with sonography is a plus.
  • Strong clinical skills in patient assessment, critical thinking, and decision-making.
  • Excellent communication skills with the ability to build rapport with patients of diverse backgrounds.
  • Knowledge of sterile processing standards is advantageous.
  • Commitment to ongoing professional development and adherence to ethical standards in healthcare.
