Nurse Practitioner (NP) – In-Home Wellness Risk Adjustment Highlights

  • The pay for this position is $120,000.00 - $ 150,000.0 annually.
  • Onsite position
  • This position is a direct hire.

Responsibilities

  • Perform annual in-home wellness visits and comprehensive risk adjustment health assessments for a population with chronic conditions.
  • Conduct physical examinations, review medication history, assess functional status, evaluate mental health, and document findings.
  • Assess pain management, cognitive status, fall risk, home environment, and daily living activities.
  • Deliver individualized patient health education on topics such as medication management, pain control, and wellness.
  • Build and maintain trusting relationships with members through exceptional communication and empathy.
  • Assist in closing quality care gaps, such as screenings, labs, and other preventive services.
  • Collaborate with other healthcare providers to enhance patient care and outcomes.
  • Travel within your assigned region to conduct in-home assessments. Travel within a 55-mile radius, with occasional out-of-area travel as scheduled.
  • Other duties as assigned to support patient care and organizational goals.

Requirements

  • Valid, unencumbered Nurse Practitioner (NP) license for the state in which you will be working.
  • Experience with in-home risk assessments or similar clinical experience is preferred.
  • At least 3 years of patient care experience, preferably in primary care, adult/geriatric care, or emergency medicine.
  • Familiarity with Electronic Medical Records (EMR) systems.
  • Ability to work independently and manage time effectively.
  • Reliable transportation for travel to patient homes within the designated area.
  • Bilingual skills are a plus.
  • Flexibility and adaptability in a dynamic healthcare environment.
