Registered Nurse (RN) – Home Health  Location: Multiple Counties, IN (Boone, Hamilton, Hancock, Hendricks, Johnson, Madison, Marion, Morgan, Shelby)
Registered Nurse (RN) – Home Health  Pay: $57.30 per hour
Registered Nurse (RN) – Home Health  Shift: Monday–Friday, 8:00 AM to 5:00 PM with rotating on-call (weekends and after-hours)
Registered Nurse (RN) – Home Health  Travel: Approximately 95% travel required
Registered Nurse (RN) – Home Health  Mileage Reimbursement: $0.50 per mile
Registered Nurse (RN) – Home Health  Overtime: Paid at 1.5x for hours over 40/week and on-call hours worked

Registered Nurse (RN) – Home Health  Position Summary

We have a high priority opening for a skilled Home Health Registered Nurse to start immediately. This role involves frequent travel to patient homes across multiple counties. The RN will assess, plan, and implement nursing care, ensuring health maintenance, disease prevention, and case management for patients in the community. Strong communication with physicians and interdisciplinary teams is critical.

Registered Nurse (RN) – Home Health  Responsibilities

• Conduct comprehensive nursing assessments and deliver care to well, chronic, or acutely ill patients in a community setting
• Collaborate with healthcare providers to develop and update individualized care plans
• Supervise licensed practical/vocational nurses and unlicensed staff as needed
• Ensure proper medication management, including ordering, monitoring, and disposal per physician orders
• Complete thorough and timely nursing documentation (daily, monthly, quarterly, and annual)
• Coordinate care transitions and maintain communication with inpatient nursing and discharge coordinators
• Provide training and recertification on health and safety topics for new and existing staff
• Maintain equipment, supplies, and environment according to infection prevention guidelines

Registered Nurse (RN) – Home Health  Requirements

• Current RN license in Indiana
• Wound Care Competency (if not currently certified, training will be provided)
• Experience with HomeCare HomeBase (HCHB) electronic medical records or willingness to complete training
• IV assessment skills required
• OASIS knowledge required; training provided if not previously completed
• Ability to travel extensively within the service counties
• CPR certification
• Valid driver’s license and insurance

Registered Nurse (RN) – Home Health  Schedule & On-Call Details

• On-call rotation schedule released 30 days in advance
• Weekend on-call triage visits as needed, with paid overtime
• Call runs Monday 5 PM through Tuesday 8 AM, plus additional weekend call as scheduled
