Registered Nurse (RN) – Clinic Setting | Travel Between Baton Rouge, New Orleans & Lafayette
Location: Baton Rouge, LA (Primary)
Travel: Up to 50% to nearby clinics in New Orleans and Lafayette
Schedule: Monday – Friday | 8:00 AM – 5:00 PM
Contract Length: Through December 31, 2025 (with potential extension)
Start Date: Targeting late September 2025
Compensation: $50/hour
Position Overview
A growing healthcare organization is seeking two Registered Nurses (RNs) to support patient care at its Baton Rouge clinic, with occasional travel to nearby locations in New Orleans and Lafayette. This role focuses on coordinating patient care, providing education on chronic conditions, and ensuring a seamless care experience for patients and their families.
The RN will work closely with an interdisciplinary care team to support patients with chronic conditions such as diabetes and cardiovascular disease, using evidence-based protocols and patient-centered care strategies. Mileage is reimbursed for travel between clinic locations.
Key Responsibilities
- Provide comprehensive nursing care through clinical assessment, patient education, and care coordination.
- Respond to patient calls and inquiries, providing triage and symptom management as needed.
- Educate patients and caregivers on chronic disease management, particularly diabetes and cardiovascular health.
- Review and update patient medications and coordinate prescription refills and prior authorizations.
- Document patient information accurately within the electronic medical record (EMR).
- Collaborate with physicians and interdisciplinary care teams to implement and monitor care plans.
- Participate in care team meetings to discuss patient progress and clinic operations.
- Conduct point-of-care testing, specimen collection, and basic clinical procedures when required.
- Support preventative care initiatives and help reduce avoidable emergency department visits.
- Deliver a high-quality patient experience through compassionate, responsive care.
Required Qualifications
- Active Registered Nurse (RN) license in Louisiana or compact state
- BLS Certification
- 3–5 years of RN experience
- Previous experience working in a clinical or outpatient setting
- Strong Electronic Medical Record (EMR) experience
- Excellent communication and patient education skills
- Strong computer and data navigation abilities
- Ability to work independently and collaborate effectively within a care team
- Ability to assess and support patients both in-person and via phone
- Authorization to work in the United States
Preferred Qualifications
- Experience supporting patients with chronic disease management
- Familiarity with medication management, prior authorizations, and prescription coordination
- Experience providing telephonic triage and patient support
- Experience working in team-based care models
