A-Line Staffing is now hiring a Registered Nurse – Home Health Hospice in Southeast Wisconsin.

RN – Home Health Hospice Compensation

  • The pay for this position is $56.14 per hour
    Mileage reimbursement at $0.53 per mile
    • Benefits are available to full-time employees after 90 days of employment
    • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

RN – Home Health Hospice Highlights

  • This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
    Schedule: Monday – Friday, 8:00 AM – 4:30 PM
    No on-call or weekend shifts required
    • Required to complete approximately 5 patient visits per day
    • Service area includes Waukesha County and Southeast Milwaukee County

RN – Home Health Hospice Responsibilities

  • Perform comprehensive assessments of high-risk members in their homes
    • Collaborate with physicians and interdisciplinary care teams to develop and implement care plans
    • Reassess patient needs to ensure appropriate and cost-effective levels of care
    • Provide education on end-of-life care, symptom management, and disease processes to patients and families
    • Coordinate non-medical services like transportation or housing to support treatment compliance
    • Work closely with social workers, discharge planners, and other providers for seamless care transitions
    • Maintain detailed documentation of all clinical, functional, and financial outcomes
    • Promote health and prevent adverse outcomes by identifying risks early and intervening when necessary
    • Serve as a patient advocate, protecting privacy and ensuring compliance with care standards

RN – Home Health Hospice Requirements

  • Active and unrestricted RN License in Wisconsin
    Wound care experience is required
    Certified BLS/CPR
    Valid driver’s license, reliable vehicle, and proof of auto insurance
    • Experience with Home Care Home Base (HCHB) is highly desired
     – MatrixCare or any EMR system experience is acceptable if no HCHB
    • Must be comfortable working in home health and/or hospice environments
    • Must complete and submit a Skills Competency Checklist during compliance
    • Attendance is mandatory for the first 90 days

Additional Compliance Requirements

  • Elder Abuse Training
    • HIPAA Training
    • Infection Prevention and Control
    • OSHA Compliance
    • Proof of education/skills training completed within the past 12 months

Preferred Qualifications

  • Associate’s or Bachelor’s degree in Nursing or a related field
    • Case Management certification
    • Ability to analyze complex medical data and make effective care decisions
    • Strong background in hospice, home health, or skilled nursing settings
