HOME HEALTH LPN | DETAILS AND COMPENSATION:

• Location: Bowling Green KY 42104 – 100% On-site
• • Counties Covered : Warren, Simpson and Logan
• Payrate: $37.30/hr + mileage, $0.49 per mile
• Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM

HOME HEALTH LPN | SUMMARY AND HIGHLIGHTS:

The Home Health LPN will deliver in-home nursing care in accordance with individualized plans of care while promoting patient dignity, independence, and safety. This role requires strong clinical skills, excellent documentation, and the ability to work independently in a home health setting.

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

HOME HEALTH LPN | RESPONSIBILITIES:

• Provide nursing care per established plans of care
• Monitor and document vital signs; report abnormal findings
• Administer medications, treatments, feedings, oxygen, and ostomy care
• Perform wound care, including wound vac management
• Assist with catheter care, drain management, and lab draws
• Follow care routines established by PT, OT, and Speech Therapy
• Maintain infection control standards and report potential infections
• Accurately document all nursing care, patient progress, and outcomes
• Order medical supplies, medications, and feedings as needed
• Ensure patient care aligns with physician orders
• Monitor incidents, complete reports timely, and assess patients post-incident
• Inspect and report faulty medical equipment for repair

HOME HEALTH LPN | REQUIREMENTS:

• Education: High School Diploma or GED (verifiable) required
• Attendance: Commitment to mandatory attendance during the first 90 days.
• Active LPN license
• Ability to travel within assigned service areas
• Strong documentation and communication skills
• Commitment to patient safety and infection control
• Reliable attendance throughout the contract duration
• Preferred: Home health experience, Wound care and wound vac experience, Familiarity with Homecare Homebase (HCHB), Experience with catheters, drains, and lab draws
