HOME HEALTH NURSE (RN) | DETAILS AND COMPENSATION:
- Location: Charlotte NC 28273 – 100% On-site
- within Mecklenburg and Union County service areas
- Payrate: $53.93/hr
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM (No weekends / No OT)
HOME HEALTH NURSE (RN) | SUMMARY AND HIGHLIGHTS:
The Home Health Nurse (RN) role focuses on care assessment, care plan implementation, health maintenance, and supporting individuals with acute, chronic, or ongoing health needs. You’ll work closely with interdisciplinary teams to ensure excellent continuity of care.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs

HOME HEALTH NURSE (RN) | RESPONSIBILITIES:
- Perform assessments and implement individualized care plans.
- Provide health maintenance, disease prevention, and case management.
- Observe, assess, plan, implement, and evaluate nursing support for individuals in community or small facility settings.
- Collaborate with physicians and healthcare professionals to coordinate care.
- Manage medication ordering, reviewing, monitoring, discontinuation, and disposal per policy.
- Document health status and maintain all required daily, monthly, quarterly, and annual nursing records.
- Monitor equipment, supplies, and infection control practices.
- Supervise and support LPN/LVNs as needed.
- Schedule and follow up on health-related appointments.
- Communicate with inpatient teams during hospitalizations and coordinate discharge follow-up.
- Train new employees and conduct annual recertification on health and safety topics.
- Provide guidance to unlicensed staff on person-specific health and safety needs.
HOME HEALTH NURSE (RN) | REQUIREMENTS:
- Education: High School Diploma or GED (verifiable)
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active RN license in North Carolina or a Compact RN license.
- Ability to work onsite Monday–Friday during standard business hours.
- Comfort providing care within Mecklenburg and Union County service areas.
