HOME HEALTH RN | DETAILS AND COMPENSATION:
- Location: Charlotte NC 28273 – 100% On-site

- Counties covered: Mecklenburg and Union County
- Payrate: $53.93/hr + $0.41 per mile
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM

- Would rotate evening or weekend call
HOME HEALTH RN | SUMMARY AND HIGHLIGHTS:
The Home Health RN will support individuals in a community-based or small facility setting. This role focuses on health assessment, care coordination, disease prevention, and case management, while collaborating closely with interdisciplinary teams to ensure high-quality patient care.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
HOME HEALTH RN | RESPONSIBILITIES:
- Assess, plan, implement, and evaluate nursing care for individuals with chronic, acute, or preventive health needs
- Coordinate care with physicians, providers, and interdisciplinary teams
- Manage medication processes including ordering, monitoring, documentation, and compliance
- Complete required daily, monthly, and annual nursing documentation
- Supervise Licensed Practical/Vocational Nurses as needed
- Train new employees and provide annual health and safety recertifications
- Ensure infection control standards, equipment maintenance, and safe care environments
- Coordinate follow-up care and communication during hospitalizations and discharges
HOME HEALTH RN | REQUIREMENTS:
- Education: High School Diploma or GED (verifiable) required
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active North Carolina RN license or Compact RN license
- Ability to work onsite
- Willingness to rotate on-call coverage as needed
- Preferred: Homecare Homebase (HCHB), Wound care and wound vac experience, IVs and port access, OASIS documentation experience
