HOME HEALTH LPN | DETAILS AND COMPENSATION:

  • Location: Charlotte NC 28273 – 100% On-site
    • Counties covered: Mecklenburg and Union County
  • Payrate: $44.03/hr + $0.41 per mile
  • Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
    • Would rotate evening or weekend call

HOME HEALTH LPN | SUMMARY AND HIGHLIGHTS:

The Home Health LPN will provide direct patient care in a community-based setting. This role focuses on delivering high-quality nursing care while supporting patient dignity, independence, and safety through consistent care coordination and documentation.

  • This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
  • Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
  • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

HOME HEALTH LPN | RESPONSIBILITIES:

  • Provide patient care in accordance with individualized plans of care
  • Monitor vital signs and report abnormal findings promptly
  • Administer medications, feedings, oxygen, and ostomy care as ordered
  • Carry out care routines established by Physical, Occupational, and Speech Therapists
  • Maintain and enforce infection control standards and procedures
  • Accurately document nursing care, patient progress, and outcomes
  • Order medical supplies and medications to ensure continuity of care
  • Monitor and report incidents, complete incident documentation, and assess patients post-incident
  • Ensure medical equipment is properly maintained and report issues for repair

HOME HEALTH LPN | REQUIREMENTS:

  • Education: High School Diploma or GED (verifiable) required
  • Attendance: Commitment to mandatory attendance during the first 90 days.
  • Active North Carolina LPN license or Compact LPN license
  • Ability to work onsite
  • Willingness to rotate on-call coverage as needed
  • Preferred: Homecare Homebase (HCHB), Wound care and wound vac experience, IVs and port access
