Hospice RN Case Manager – Multiple Counties in TN (Morgan, Roane, Rhea, Bledsoe, Loudon)

A-Line Staffing is now hiring a Hospice RN Case Manager in Tennessee. This is a high-priority, full-time opportunity offering mileage reimbursement, a Monday–Friday schedule, and on-call rotation every 6 weeks. Hospice experience is required. RN Case Manager Compensation

• The pay for this position is $56.14 per hour
  • Mileage Reimbursement: $0.41 per mile
  • Benefits are available to full-time employees after 90 days of employment
  • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Schedule & Coverage Area

• Monday–Friday, 8:00 AM – 4:30 PM
  • On-call rotation every 6 weeks, Monday to Monday, from 4:30 PM – 8:00 AM
  • Travel required to Morgan, Roane, Rhea, Bledsoe, and Loudon counties
  • 25 points per week expectation

RN Case Manager Responsibilities

• Perform comprehensive assessments of high-risk patients in hospice care
  • Collaborate with primary care providers to create and implement individualized care plans
  • Address and reassess the appropriateness and cost-efficiency of care provided
  • Put non-medical supports in place to encourage compliance (e.g., housing, transportation)
  • Coordinate care transitions and work with interdisciplinary teams
  • Engage and consult with physicians, social workers, and discharge planners
  • Document detailed records of clinical, functional, and financial outcomes
  • Identify and act on opportunities for health promotion and illness prevention
  • Intervene quickly to minimize adverse outcomes and advocate for patient safety and dignity

RN Case Manager Requirements

• Active RN License for the State of Tennessee (or compact license)
  • Associate’s or Bachelor’s Degree in Nursing
  • Valid driver’s license, auto insurance, and reliable transportation
  • Hospice experience required
  • CPR certification
  • Attendance is mandatory for the first 90 days

Preferred Qualifications

• Experience using HCHB (Homecare Homebase)
  • Case Management Certification (CCM, ACM, or equivalent)
  • Strong clinical decision-making and communication skills
