Ambulatory Case Manager (RN or LMSW) – Remote | $37/hr | Monday–Friday 8AM–5PM

A-Line Staffing Solutions is now hiring Ambulatory Case Managers (RN or LMSW) for a remote role supporting a leading healthcare organization’s ambulatory case management team. Position Highlights:

• Pay Rate: $37.00 per hour

• Remote/WFH – Must live in the Detroit, MI area (unless pre-certified in Red Cross billing/coding)

• Schedule: Monday – Friday | 8:00 AM – 5:00 PM

• Contract with potential to extend or convert to permanent after 3 - 6 months 

What You’ll Be Doing:

• Provide  telephonic case management to primary care patients.

• Assess patient needs, set goals, and build care plans tailored to individual health journeys.

• Identify and connect patients to community and health resources using SDoH assessments.

• Collaborate with multidisciplinary teams and support care coordination efforts.

• Utilize Epic’s Compass Rose system (training provided) for documentation and tracking.

• Complete Red Cross billing/coding training within 6 months of start (unless already certified).

Required Qualifications:

• Active Michigan RN license or LMSW license in good standing.

• Bachelor’s in Nursing or Master’s in Social Work required.

• Minimum 3 years of clinical experience and 3–5 years in telephonic case management.

• Familiar with:

  • Motivational interviewing

  • Goal setting and care planning

  • Care coordination and resource navigation

  • Case management billing codes and documentation standards

• Comfortable working independently in a fast-paced, performance-driven virtual environment.

Preferred Qualifications:

• Experience working in primary care clinics or ambulatory settings.

• Familiarity with Patient Centered Medical Home (PCMH) and Provider Delivered Care Management (PDCM) billing.

• Prior training through MICMT (e.g., “Introduction to Team-Based Care” or “Patient Engagement Foundations”).

• Epic EMR experience is a plus.
