Hospice RN Case Manager – Coffee, Franklin, Grundy & Van Buren Counties, TN

A-Line Staffing is now hiring a Hospice RN Case Manager to provide comprehensive care coordination and case management in Coffee, Franklin, Grundy, and Van Buren counties, Tennessee. This is a full-time, onsite contract role with mileage reimbursement and a competitive hourly rate.

Compensation

• Pay rate: $56.14 per hour
• Mileage reimbursement: $0.41 per mile
• Benefits available to full-time employees after 90 days of employment
• 401(k) with company match after 1 year of service on eligibility dates

Schedule

• Monday – Friday, 8:00 AM to 4:30 PM
• Participate in scheduled on-call rotation (schedule released 30 days in advance)
• Approximately 25 patient visits per week (multiple sites per day)

Requirements

• Active, unrestricted RN license in the state of Tennessee or compact state
• Associates or Bachelor’s Degree in Nursing or related field
• Wound care experience required (no certification necessary)
• Experience with HomeCare HomeBase (HCHB) is required
• Current CPR/BLS certification
• Valid driver’s license and insurance

Primary Responsibilities

• Perform comprehensive assessments of high-risk patients
• Collaborate with primary care providers to develop and implement individualized care plans
• Continuously reassess and address appropriate, cost-effective levels of care
• Arrange non-medical support (housing, transportation) to ensure treatment compliance
• Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals
• Engage specialty resources to optimize patient outcomes
• Maintain detailed clinical, functional, and fiscal records throughout case management
• Identify opportunities for health promotion and illness prevention
• Intervene promptly to prevent adverse patient outcomes when possible
• Act as patient advocate, ensuring privacy and confidentiality
