LPN/LVN (Home Health)-{170721}
Location: Lexington, KY 40505
Pay: $42/hour
Schedule: 40 hrs./week, rotating weekends; possible on-call
Position Overview
We are seeking an LPN/LVN to provide skilled nursing care in patients’ homes under the supervision of an RN Case Manager. You will support patient assessments, coordinate care with the interdisciplinary team, and document care in compliance with State, Federal, and Medicare standards.
Key Responsibilities
- Provide skilled nursing care in the home per physician orders and plan of care
- Identify patient physical, psychosocial, and environmental needs through observation and documentation
- Coordinate care and report outcomes to the RN Case Manager
- Educate patients and families on self-care techniques and assess response to care
- Document interventions and outcomes clearly in clinical notes and communication logs
- Follow infection control and safety standards (OSHA)
- Support end-of-life care activities as assigned (Hospice-related support may apply)
- Assist with admissions, discharges, and transfers as needed
Job Requirements
Required Qualifications
- Active LPN/LVN license in the practicing state
- Current BLS/CPR certification
- Valid driver’s license, reliable transportation, and current auto insurance
- 2+ years LPN/LVN experience (home health or hospice)
- Proof of Hepatitis consent/declination and TB vaccination record
- OASIS & Homecare Homebase experience
