Hospice RN Case Manager Compensation
• The pay for this position is $56.14
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Hospice RN Case Manager Highlights
• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• The required schedule is Monday – Friday, 8:00 AM – 4:30 PM
• No weekend or on-call work required
• Counties covered: primarily Dauphin and Cumberland; may be asked to assist in Lebanon, Perry, Franklin, and Adams Counties
• Office is based in Dauphin County
• Productivity benchmark is 5 to 7 visits per day
Hospice RN Case Manager Responsibilities
• Perform admission, recertification, routine, PRN, and death visits for hospice patients
• Conduct comprehensive assessments of high-risk members and collaborate with primary care providers
• Develop and implement individualized care plans to ensure patient engagement and adherence
• Reassess care plans to ensure cost-efficiency and appropriateness of care level
• Coordinate non-medical support (e.g., housing, transportation) to support treatment compliance
• Facilitate care transitions in collaboration with physicians, discharge planners, and other healthcare professionals
• Maintain thorough documentation of clinical, functional, and fiscal outcomes
• Identify opportunities for health promotion and prevention of illness
• Act as a patient advocate, maintaining privacy and confidentiality
Hospice RN Case Manager Requirements
• Active and unrestricted RN license in the state of Pennsylvania
• Associate’s or Bachelor’s degree in Nursing or a related field
• Valid driver’s license, car insurance, and reliable transportation
• Current BLS/CPR certification
• Hospice and HCHB (Homecare Homebase) experience required
• Case Management certification preferred
• Ability to analyze complex medical information and make sound clinical decisions
• Attendance is mandatory for the first 90 days
