A-Line Staffing is now hiring Registered Nurses (Hospice / Home Health) in Fond du Lac, Dodge, and Columbia Counties, WI. The Hospice RN would be working for a Fortune 500 company and has career growth potential. This would be full time / 40+ hours per week.

Registered Nurse (Hospice / Home Health) Compensation
The pay for this position is $56.14 per hour.

Benefits are available to full-time employees after 90 days of employment.
A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates.

Registered Nurse (Hospice / Home Health) Highlights

  • This position is a contract assignment with potential to hire permanently based on performance and business needs.
  • Shift: Monday–Friday, 8:00am–4:30pm — No weekends or on-call!
  • Visits: 5 visits per day on average.
  • Mileage Reimbursement: $0.53 per mile.
  • Counties Covered: Fond du Lac, Dodge, and Columbia Counties.
  • Additional patient coverage in Jackson, Waterford, and Sheboygan.
  • Training Pay: $60/hour during training, then $56.14/hour after full onboarding completion.
  • Candidates without HCHB or Hospice experience will receive paid training (16+ hours depending on skill level).

Registered Nurse (Hospice / Home Health) Responsibilities

  • Perform comprehensive assessments for high-risk patients in home settings.
  • Complete admission, recertification, routine, PRN, and death visits.
  • Develop and implement individualized care plans with interdisciplinary teams.
  • Coordinate care transitions and community resource support.
  • Collaborate with physicians, social workers, chaplains, CNAs, and family members.
  • Manage pain, wound care, and symptom control with compassion and precision.
  • Document care accurately in EMR systems (HCHB, MatrixCare, or similar).
  • Maintain compliance with hospice standards, OSHA, HIPAA, and infection control policies.

Registered Nurse (Hospice / Home Health) Requirements

  • Active RN License in Wisconsin.
  • Associate or Bachelor’s Degree in Nursing.
  • Certified CPR/BLS (current).
  • Valid driver’s license, reliable vehicle, and active car insurance.
  • Wound care experience required.
  • Electronic Medical Record experience: HCHB highly preferred; MatrixCare or other EMR acceptable.
  • Completion of the following (required for compliance):
    • Skills competency checklist
    • Elder abuse training
    • HIPAA training
    • Infection prevention and control
    • OSHA compliance
    • Documentation of any clinical education or training completed in the past 12 months
  • High School Diploma or GED.
  • Attendance is mandatory for the first 90 days.

Registered Nurse (Hospice / Home Health) Preferred Qualifications

  • Case Management experience or certification (CCM/ACM).
  • Previous hospice, home health, or skilled nursing experience.
  • Experience providing education to families and staff on end-of-life processes.
  • Knowledge of IDG documentation, recertification, and care planning.
