Senior Field RN Care Manager Compensation
The pay for this position is $35.49 – $63.79 per hour based on experience
- 8% annual bonus incentive
- Mileage reimbursement provided
- Equipment provided
- Benefits are available to full-time employees after 90 days of employment
- A 401(k) with a company match is available for full-time employees
Senior Field RN Care Manager Highlights
- Monday–Friday schedule (8:30am – 5:00pm)
- Field-based role conducting 2–4 UAS assessments daily
- Must reside in Albany or Rensselaer County, NY
- Permanent, direct hire opportunity
Senior Field RN Care Manager Responsibilities
- Conduct UAS assessments and reassessments for LTSS members
- Develop, implement, and manage individualized care plans
- Coordinate care across providers, caregivers, and community resources
- Monitor member progress and adjust care plans as needed
- Educate members and families on services, benefits, and care options
- Collaborate with interdisciplinary care teams to ensure quality outcomes
- Maintain accurate documentation in compliance with state and federal guidelines
- Identify high-risk members and support appropriate level-of-care transitions
Senior Field RN Care Manager Requirements
- Active New York RN license
- 4+ years of RN experience
- UAS assessment experience required
- Ability to perform field-based home/site visits
- Strong care management and coordination experience
- Valid driver’s license (personal transportation preferred)
Preferred Qualifications
- Bachelor’s degree in Nursing (BSN)
- Experience in Managed Care / MLTC / LTSS
- Background working with high-acuity populations
- Knowledge of community resources and care coordination
