HOME HEALTH LPN | DETAILS AND COMPENSATION:
- Location: Bergen County, NJ – 100% On-site, home health settings
- Payrate: $37.30/hr + mileage
- Required Availability: Full-Time | Monday – Friday, 8:00 AM–5:00 PM + weekends & on-call rotation

- Minimum productivity benchmark of 30 points per week (may vary based on patient needs)
- weekly case conferences (Wednesdays @ 8:45 AM, approx. 1.5 hours)
HOME HEALTH LPN | SUMMARY AND HIGHLIGHTS:
The Home Health LPN will provide skilled home care services while supporting patients in maintaining dignity, independence, and quality of life.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
HOME HEALTH LPN | RESPONSIBILITIES:
- Deliver care in accordance with the patient’s established plan of care
- Monitor and document vital signs, reporting abnormal findings promptly
- Administer medications, feedings, oxygen therapy, and ostomy care as needed
- Provide skilled nursing services including wound care, wound vac management, and Foley catheter care
- Collaborate with interdisciplinary teams (PT, OT, Speech Therapy) to implement care routines
- Maintain strict infection control standards and report communicable concerns
- Complete accurate, timely documentation (within 24 hours)
- Participate in weekly case conferences (Wednesdays @ 8:45 AM, approx. 1.5 hours)
- Order and manage necessary medical supplies and medications
- Monitor and report incidents, ensuring appropriate follow-up and documentation
- Ensure medical equipment is functioning properly and report issues as needed
HOME HEALTH LPN | REQUIREMENTS:
- Education: High School Diploma or GED (verifiable)
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Timely documentation and consistent attendance at required meetings
- Active Licensed Practical Nurse (LPN) license
- Reliable transportation for commuting within coverage area
- Strong documentation and communication skills
Preferred Skills
- Skilled nursing experience in a home care setting
- Experience with wound care, wound vacs, Foley catheter management, and ostomy care
- Proficiency with electronic documentation systems (Home Care Home Base preferred)
- Comfortable using tablets, point-of-care technology, and Microsoft Teams
