Hospice / Outpatient Bereavement Coordinator Position Overview
Seeking a Hospice/Outpatient Bereavement Coordinator to support patients and families coping with end-of-life and grief-related needs. This role is part of a multidisciplinary hospice team providing compassionate emotional support, grief counseling, and bereavement services across home and community-based settings.
Hospice / Outpatient Bereavement Coordinator Department Overview
The hospice program supports patients and families through the final stages of illness across Linn, Benton, Marion, Polk, Lincoln, and Tillamook counties. Care is delivered in home environments including private residences, skilled nursing facilities, assisted living communities, and adult foster homes. Services include skilled nursing, symptom management, social services, counseling, spiritual care, hospice aides, volunteers, and structured bereavement support. Medications and medical equipment are provided as appropriate.
Hospice / Outpatient Bereavement Coordinator Key Responsibilities
- Provide grief and loss counseling to patients’ families and caregivers
- Coordinate and deliver outpatient bereavement services and follow-up programs
- Facilitate individual and group grief support sessions
- Collaborate with interdisciplinary hospice team members
- Maintain accurate documentation of counseling and bereavement activities
- Participate in care planning and family support interventions
Required Qualifications
- Bachelor’s degree in Social Services, Counseling, Psychology, or related field (Master’s preferred)
- Experience or formal education in grief or loss counseling
- Healthcare Provider Level BLS certification
- Valid driver’s license with auto liability/property damage insurance
- Reliable personal vehicle in good working condition
Hospice / Outpatient Bereavement Coordinator Preferred Qualifications
- Recent hospice experience
- Experience in a medical or healthcare setting
Hospice / Outpatient Bereavement Coordinator Work Environment
This position requires travel to patient homes and care facilities within the service area. The role involves direct interaction with individuals and families experiencing grief and end-of-life transitions.
