Job Description:
- Support patient assessment and prioritization for telephonic triage to ensure appropriate care and prevent unnecessary hospital visits.
- Execute protocol-driven chronic condition management.
- Provide comprehensive patient and caregiver education regarding chronic conditions and preventative care over the phone.
- Conduct medication reviews within nurse-led visits to ensure patient safety and adherence.
- Review and act upon abnormal laboratory results to maintain patient safety and continuity of care.
- Utilize standardized protocols for medication management, prescription renewals, and prior authorizations.
- Manage clinical communications, ensure task completion, and close patient visit encounters promptly.
- Remote work environment
- Supports patient assessments, medication management, and clinical documentation
- Ensures clinical compliance with safety initiatives and reporting
- Provides emergency care as per organizational procedures
- Active, Unrestricted Registered Nurse Licensure in Illinois
- BLS or CPR Certification
- Proficiency with EMR systems; Epic experience preferred
- Fluent in English; bilingual (Spanish) skills preferred
