Job Description
- Support patient assessment and prioritization for telephonic triage to ensure appropriate care and reduce unnecessary hospital visits.
- Execute protocol-driven chronic condition management.
- Provide comprehensive patient and caregiver education regarding chronic conditions and preventative care over the phone.
- Conduct medication reviews to ensure patient safety and adherence.
- Review and respond to abnormal laboratory results promptly to maintain patient safety and continuity of care.
