HOME HEALTH OCCUPATIONAL THERAPIST | DETAILS AND COMPENSATION:
- Location: Cranston, RI 02907 – 100% On-site/Fieldwork
- Payrate: $55.61/hr
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
HOME HEALTH OCCUPATIONAL THERAPIST | SUMMARY AND HIGHLIGHTS:
The Home Health Occupational Therapist role focuses on providing high-quality Occupational Therapy services within a home-care setting. The ideal candidate brings strong assessment skills, solid home-care experience, and confidence managing a full caseload while collaborating across disciplines.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs

HOME HEALTH OCCUPATIONAL THERAPIST | RESPONSIBILITIES:
- Perform comprehensive assessments of patient needs, home environments, and support systems to create individualized plans of care.
- Implement physician-ordered Occupational Therapy services, ensuring all care meets professional, regulatory, and ethical standards.
- Identify therapy diagnoses, update plans of care, and document patient progress thoroughly and accurately.
- Coordinate treatment with other disciplines to ensure integrated, patient-centered care.
- Provide education to patients and caregivers on diagnosis, medications, safety, infection control, and therapeutic exercises.
- Conduct medication reviews and ensure patients/caregivers understand usage, side effects, and contraindications.
- Manage equipment and supply requisitions to support patient function and independence.
- Supervise OT assistants and home health aides as needed.
- Maintain compliance with state, payer, and agency requirements, including timely and accurate documentation and OASIS proficiency.
- Adhere to infection control protocols, maintain universal precautions, and ensure proper sanitizing/transport of equipment.
- Recognize and report changes in patient status and incidents per agency guidelines.
- Participate in meetings, care conferences, QAPI activities, and ongoing training.
HOME HEALTH OCCUPATIONAL THERAPIST | REQUIREMENTS:
- Education: High School Diploma or GED (verifiable)
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active Rhode Island OT license or Compact OT license.
- Home care experience is required.
- Strong clinical assessment and documentation skills.
- Ability to manage a dynamic caseload and collaborate effectively with interdisciplinary teams.
- Proficiency with OASIS documentation strongly preferred.
