A-Line Staffing is now hiring Patient Care Advocate (HEDIS Care Gaps) in Toledo or Dayton, OH (must reside within an hour).
The Patient Care Advocate would be working for a Major Health Plan and has career growth potential.
Patient Care Advocate (HEDIS Care Gaps) Highlights:
- Schedule: 9:00 AM – 5:00 PM EST (Various shifts to support later outreach hours)
- Location: Must reside in Toledo or Dayton, Ohio; mileage covered for travel to provider offices within an hour
- Onsite Role: Yes – in provider offices
- Pay Rate: $25-32/hour
Patient Care Advocate (HEDIS Care Gaps) Responsibilities:
- Conduct coordinated outreach to address and close HEDIS care gaps
- Assist members with removing barriers to care, such as appointment scheduling, transportation, and benefit education
- Provide information on preventative care and healthy rewards
- Communicate daily with members and providers to support quality improvement initiatives
- Work both independently and collaboratively in the field and remote settings
- Visit local provider offices as needed (within 1 hour of home)
- Track and report progress using Centene systems and tools
Patient Care Advocate (HEDIS Care Gaps) Qualifications:
- Required Experience:
- Customer service experience
- At least 2 years in healthcare or quality-focused roles
- Preferred Experience:
- Provider engagement or representative background
- Clinical credentials (RN, LPN, Social Worker)
- Education Required:
Bachelor’s degree in Healthcare, Public Health, Nursing, Psychology, Social Work, Health Administration, or 2+ years of equivalent experience in a health-related field - Education Preferred:
Bachelor’s Degree - Software Skills Required:
Proficiency with Microsoft Office Suite (Excel, Outlook, Word)
