Job Description
- Conduct a comprehensive clinical review of pharmacotherapy for each patient to ensure treatment plans align with evidence-based recommendations.
- Document results of pharmacotherapy reviews to inform the nursing team for follow-up assessments.
- Counsel patients on new drug therapy to educate them on their planned treatments.
- Coordinate patient care with referring clinicians, addressing social, psychological, and physical needs that require customized services.
- Dispense all medications pursuant to physician orders in compliance with state and federal laws.
- Monitor hazardous waste material management within the pharmacy and ensure proper handling of controlled substances.
- Support professional development goals of staff and maintain ongoing education within the pharmacy arena.
Critical Information
- Onsite position, Monday to Friday, from 9 AM to 6 PM.
- Must be licensed in both California and Kentucky, with licenses in good standing.
- Must live within one hour driving distance of the job location.
- Must enjoy being on the phone for long periods.
Education/Licenses Needed
- Graduate of an accredited school of pharmacy.
- Current and unrestricted pharmacy license in good standing.
- 3-5 years of post-graduate experience in pharmacy operation, clinical oversight, and patient care.
- BCOP certification or willingness to become certified when eligible.
