Job Description:
- Provide direct and indirect patient care services to individuals in their home or alternative settings following organizational policies and physician-approved plans of care.
- Evaluate patient needs and develop individualized treatment programs tailored to each patient.
- Administer complex therapeutic exercises to improve patient mobility and daily activities.
- Instruct, motivate, and assist patients and caregivers in enhancing functional mobility and activities of daily living.
- Supervise and oversee Physical Therapist Assistants and Home Health Aides, ensuring quality standards are met.
- Shift: Monday-Friday, 8 am-5 pm
- On-site at Lancaster, SC
- No on-call schedule
- Compliance with organizational policies and physician plans
- Effective communication with interdisciplinary team
- Physical Therapist license
- Prior home health experience
- Experience with HCHB system
