Job Description
- Perform physical therapy evaluation and treatment of patients based on physician orders/referrals.
- Complete comprehensive evaluations using appropriate assessment tools and procedures.
- Establish measurable goals and develop treatment plans to maximize patient independence and function.
- Regularly reassess treatment effectiveness and modify goals or treatment as needed.
- Consult with medical providers regarding patient progress and discharge from treatment.
- Maintain and submit evaluation, treatment, and billing documentation as required.
- Supervise Physical Therapist Assistants and Aides in accordance with state practice act.
- M-F, 8 AM to 5 PM, 40 hours guaranteed.
- On-site position.
- Counties covered: Bartow and Paulding Counties.
- Physical Therapist License required.
- Experience with HCHB and OASIS preferred.
- Benefits are available to full-time employees after 90 days of employment.
