HOME HEALTH PHYSICAL THERAPIST | DETAILS AND COMPENSATION:
- Location: Covington GA 30014 – 100% On-site, home health settings
- Payrate: $57.30/hr + mileage
- Required Availability: Full-Time | Monday–Friday
HOME HEALTH PHYSICAL THERAPIST | SUMMARY AND HIGHLIGHTS:
The Home Health Physical Therapist will provide high-quality, patient-centered care in a home health setting. This role focuses on evaluating patients, developing individualized treatment plans, and supporting patients in achieving their highest level of independence and functional mobility.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
HOME HEALTH PHYSICAL THERAPIST | RESPONSIBILITIES:
- Perform physical therapy evaluations and treatments based on physician orders and referrals
- Complete comprehensive assessments using appropriate evaluation tools and procedures
- Review patient medical history, diagnoses, precautions, and contraindications
- Establish measurable goals and develop individualized treatment plans
- Reassess patient progress regularly and modify treatment plans as needed
- Communicate patient progress and needs with physicians, care teams, patients, and family members
- Consult with medical providers regarding treatment progress and discharge planning
- Recommend and assist with securing adaptive equipment to improve patient function
- Maintain accurate and timely clinical and treatment documentation in compliance with regulations
- Supervise Physical Therapist Assistants and Therapy Aides per state practice act
- Participate in staff development and continuing education activities
HOME HEALTH PHYSICAL THERAPIST | REQUIREMENTS:
- Education: High School Diploma or GED (verifiable)
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active Georgia Physical Therapist license
- Degree in Physical Therapy from an accredited program
- Ability to work independently and as part of an interdisciplinary team
- Strong communication and documentation skills
Preferred Skills
- Home health experience
- Experience with HCHB documentation systems
- Strong patient education and care coordination skills
