PHYSICAL THERAPIST - HOME HEALTH | DETAILS AND COMPENSATION:
- Location: De Queen AR 71832 – 100% On-site
- Payrate: $74.15/hr + $0.49 a mile
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
PHYSICAL THERAPIST - HOME HEALTH | SUMMARY AND HIGHLIGHTS:
The Physical Therapist will provide comprehensive care coordination for high-risk members. This role involves clinical assessment, care planning, and collaboration with interdisciplinary teams to ensure appropriate, cost-effective care while supporting positive patient outcomes.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance

PHYSICAL THERAPIST - HOME HEALTH | RESPONSIBILITIES:
- Perform physical therapy evaluations and treatments based on physician orders/referrals
- Complete comprehensive assessments using appropriate evaluation tools and procedures
- Review patient records for diagnoses, medical history, precautions, and contraindications
- Establish measurable goals and develop individualized treatment plans
- Regularly reassess treatment effectiveness and adjust goals or plans as needed
- Communicate patient progress and needs with physicians, care teams, and family members as appropriate
- Consult with medical providers regarding treatment progress and discharge planning
- Recommend and assist with adaptive equipment to improve patient function
- Maintain timely and accurate evaluation, treatment, and billing documentation in compliance with regulations
- Adhere to professional standards, company policies, and applicable state and federal laws
- Supervise Physical Therapist Assistants and Physical Therapy Aides in accordance with state practice acts
- Participate in staff development, meetings, and continuing education as required
PHYSICAL THERAPIST - HOME HEALTH | REQUIREMENTS:
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active Arkansas Physical Therapist license (compact license accepted)
- Ability to work independently while collaborating with a multidisciplinary care team
- Strong clinical assessment, communication, and documentation skills
- Preferred: Experience with OASIS documentation, Familiarity with PointCare
