PHYSICAL THERAPIST | DETAILS AND COMPENSATION:
- Location: Laurens County – 100% On-site, home health settings
- Payrate: $64.04/hr
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM

- Weekends/Holidays: Some weekend rotation may be required; no holidays or on-call requirements.
PHYSICAL THERAPIST | SUMMARY AND HIGHLIGHTS:
The Physical Therapist will perform evaluations and provide high-quality treatment based on physician referrals. The ideal candidate will develop comprehensive, measurable treatment plans designed to help patients reach their maximum level of independence and function.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
PHYSICAL THERAPIST | RESPONSIBILITIES:
- Evaluation & Treatment: Conduct comprehensive assessments using appropriate tools to develop and implement individualized treatment plans.
- Goal Management: Establish measurable functional goals and regularly re-assess patient progress, modifying plans as necessary.
- Collaboration: Communicate effectively with medical providers, family members, and the treatment team regarding patient status and discharge planning.
- Supervision: Oversee Physical Therapist Assistants and Aides in accordance with state practice acts.
- Documentation: Maintain timely and accurate billing and clinical documentation in alignment with company, state, and federal regulations (HIPAA).
- Adaptive Equipment: Recommend and assist in securing special equipment to improve patient mobility and function.
PHYSICAL THERAPIST | REQUIREMENTS:
- Education: High School Diploma or GED (verifiable)
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Licensure: Active Georgia State Physical Therapist license or a valid Compact license is required.
- Experience: Previous experience in a similar clinical setting is preferred.
- Preferred Skills: Familiarity with HCHB (Homecare Homebase) and OASIS documentation is highly preferred (but not a dealbreaker).
- Compliance: Ability to adhere to professional practice standards and corporate compliance programs.
